Important! Live Life Well Incentive information


Dear State of Montana Benefit Plan Members,

If you want to earn your full Live Life Well Incentive, you must self-report your Next Steps Activity and/or Nicotine Free Alternative (if it applies) by October 31, 2016 at www.myactivehealth.com/som.

We know many members have had trouble reporting their Incentive information.

Please carefully read the Steps and Troubleshooting below to be sure you have correctly reported your information.



Steps:

  1. Click “My Rewards.
  2. Scroll down to “Next Steps Activity.”
  3. Click “Work On It.”
    1. If it says, “Result,” you’ve already reported your activity and do not need to do anything else.
  4. Click the box of the Next Steps activity you completed under the “Report Activity” column.
    1. You must self-report in the Next Steps Activity Section even if you completed the Governor’s Stay Active Challenge, Strive For Five, Healthy Bingo or the Holiday Challenge and those sections say, “Completed.”
    2. If you had no At-Risk levels and completed Strive For Five, Healthy Bingo or the Holiday Challenge select “Other Live Life Well Challenges” at the bottom of the list.
  5. Click “Save.”



Troubleshooting:

  • For assistance with the www.myactivehealth.com/som portal, call (855) 206-1302.
  • Once you save your activity, your Reward progress and dollar amount at the top may not look correct right away.
  • If your health screening is showing in the portal, it can take up to 72 hours to update.
  • It can take up to a month for your State-sponsored health screening to show in the portal.
    • If it’s been more than a month and your health screening and/or Nicotine Free status aren’t appearing correctly, please email livelifewell@mt.gov.  
    • If you have a State-sponsored health screening through Care Here between November 1, 2015 and October 31, 2016, you WILL get credit for it even if it doesn’t show correctly in the portal.
  • Do not wait for your health screening to show in the portal. SELF REPORT your Next Steps Activity now! Your Next Steps Activity and/or Nicotine Free Alternative will be saved and will update automatically when your screening is in the system.
  • You cannot uncheck a Next Step Activity selection once you’ve saved it. It is ok to report more than one activity.

Schedule Your Health Screening Now!


Dear State Benefit Plan Member,Don’t miss your opportunity for a health screening. Schedule your health screening at a Montana Health Center or off-site screening today! 

Appointments are filling up fast because more people are having their State-sponsored health screening earlier in the year.
  • The health screening is the first step toward completing your Live Life Well Incentive.
  • All requirements must be completed October 31, 2016 to get the full Incentive.
  • More coming soon on how to track and report your Incentive activities.
Appointments are required:
  • Log in at www.carehere.com or call (800) 200-6822.
  • First time making an appointment with CareHere—Register with access code MANA9.
  • Select “Blood Draw” next to “What and type Health Screening in the notes box
  • For Off-site Screenings, select the exact date of the screening you want next to “When,” and “Off-Site Health Screenings” next to “Where.”

The State Plan covers one screening each plan year for members of 18 or older.

The State Plan offers the incentive program to all plan members and their enrolled spouse/domestic partner.  If you think you may be unable to meet a standard of the incentive program, you may qualify for an alternative program or different means to earn the incentive. You must contact the Health Care & Benefits Division (HCBD) as soon as possible at 800-287-8266 or email livelifewell@mt.gov.  We will work with you (and if you wish, your doctor) to design a program with the same incentive that is right for you.  Any personal medical information gathered during the course of the incentive program is protected by and will be treated consistent with the HIPAA Privacy and Security Rules.  A copy of the Plan’s privacy notice is available HERE
