In addition to completing your State-sponsored health screening, you must complete AND SELF-REPORT nicotine and next step activities to earn ANY Live Life Well Incentive for 2020 by October 31, 2019.

Dear State Plan Member,

If you want to earn a Live Life Well Incentive for 2020, you must complete ALL THREE activities to earn ANY incentive.

1. Complete your State-sponsored health screening (remember, this will automatically show up on MyActiveHealth about a month after you complete your State-sponsored screening); AND
2. Self-Report completion of an eligible Next Step activity; AND
3. Self-Report Nicotine-Free status or an eligible alternative.
